Registering with the surgery
There are two ways for you to register with the surgery. You may come to the surgery to collect our on-site registration forms. Alternatively you may register using our new Online Registration forms using the link below:
Register with The Oakley Surgery
To register with the surgery, we ask for you to provide us with two forms of identity. You will need:
-Photo Identification (Such as driving licence/passport/UK ID Card
-Proof of current address with your name on (such as a utility bill, residential lease, property deed or bank statement)
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
